Дегтярное масло, сложное вещество, получаемое путем перегонки древесного дегтя, является еще одной пирогенной жидкостью, обладающей ядовитыми свойствами. Г-да Слейт из Портсмута описали случай с моряком, который, приняв по ошибке почти четыре унции вместо спиртного, начал часто рвать массой с сильным запахом дегтя, сопровождавшейся чрезмерной болью в кишечнике и пояснице. Легочная слизистая оболочка была сильно инъецирована, легкие переполнены кровью и имели запах дегтя, желудок и кишечник были в норме, за исключением того, что все valvulae conniventes были желтыми, мозг и его оболочки также были в норме. Он был предметом физиологических экспериментов различных исследователей, особенно д-ра Кормака, который обнаружил, что дозы в двадцать пять или сорок капель вызывали смерть через несколько секунд при введении в яремную вену собаки, останавливая сердечную деятельность и не изменяя заметно состояние крови; что несколько большее количество вызывало лишь сонливость и судорожные подергивания мышц при введении в сонную артерию, не приводя к смертельному исходу; что тридцать капель, введенные в желудок кролика, вызывали судороги, острые крики и смерть через одну минуту, по-видимому, от остановки сердечной деятельности; и что та же доза, данная собаке, вызывала слюнотечение, головокружение, тетанический спазм, слабый, трепещущий, почти незаметный пульс и общую потерю сознания с расширенными неподвижными зрачками; однако выздоровление наступило после применения кровопускания. [2556] Результаты экспериментов д-ра Кормака на животных приводят к выводу, что при отравлении креозотом это вещество всегда можно обнаружить в организме, если оно не было удалено искусственным путем за значительное время до смерти.

ГЛАВА XLII. О КОМБИНИРОВАННОМ ОТРАВЛЕНИИ.

Исследовав три великих класса ядов в их отношении к физиологии, врачебной практике и судебной медицине, необходимо сделать несколько замечаний по предмету, имеющему значительное судебно-медицинское значение, который был почти упущен в системах токсикологии — комбинированному отравлению.

Когда два яда с различными или противоположными свойствами вводятся примерно в одно и то же время в ядовитых дозах, действие одного может подавить и предотвратить действие другого, или они могут просто изменить действие друг друга. Таким образом, обычные симптомы, вызываемые одним или обоими ядами, могут полностью или в значительной степени отсутствовать; и даже в трупе обычные признаки, вызванные одним или обоими ядами, могут быть изменены или, возможно, полностью отсутствовать.

Хотя в процессе чтения я встретил достаточное количество подобных случаев, чтобы показать, что комбинированное отравление является вопросом, имеющим некоторое значение для судебного медика, факты, обнародованные до сих пор, не столь многочисленны, чтобы сделать возможной их систематическую классификацию. Поэтому наиболее целесообразным представляется просто описать на данный момент случаи, которые были доведены до моего сведения. Они заключаются в следующем:

1. Отравление мышьяком и алкоголем. — Мужчина, выпив двенадцать унций виски во время попойки, час спустя, находясь в состоянии возбуждения, но не будучи особенно пьяным, проглотил некоторое количество мышьяка, дозу которого установить не удалось. Через пятнадцать минут после приема мышьяка была получена медицинская помощь, после чего были предприняты неоднократные попытки вызвать рвоту с помощью ипекакуаны и сульфата цинка, но безуспешно. Этим случаем я обязан своему бывшему ученику, г-ну Кингу.

Отравление мышьяком и алкоголем. — Случай того же описания, что и предыдущий, но закончившийся смертельным исходом вследствие большого количества принятого мышьяка, был описан д-ром Вудом из Дамфриса. Семнадцатилетний юноша после ночной попойки рано утром проглотил пол-унции мышьяка. Через два с половиной часа, когда д-р Вуд впервые увидел его, не было никаких симптомов отравления мышьяком — на самом деле, не было никаких симптомов, кроме вялости и сонливости. Через несколько минут у него началась легкая рвота, которая неоднократно возобновлялась искусственными средствами. В течение нескольких часов пульс был лишь немного повышен. Через восемнадцать часов он начал угасать и проявил обычные конституциональные симптомы отравления мышьяком; и через сорок один час он скончался. Но от начала до конца у него почти не было местных симптомов, кроме рвоты, даже несмотря на то, что желудок после смерти показал признаки сильного раздражения. [2557]

3. Отравление рвотным камнем и парами древесного угля. — В разделе об отравлении сурьмой уже упоминался случай с мужчиной, который, проглотив семнадцать гран рвотного камня, попытался покончить с собой, задушив себя парами горящего древесного угля. Он оправился от обеих попыток, сильно пострадал от обычных наркотических эффектов углекислого газа, но почти не проявил симптомов раздражающего действия рвотного камня. [2558]

4. Отравление алкоголем и лауданумом. — В разделе об отравлении опиумом уже упоминался примечательный случай, описанный г-ном Ширманом, когда обычные эффекты опиума были значительно замедлены у человека, который в момент проглатывания опиума находился в состоянии возбуждения от опьянения. В течение пяти часов не было существенного ступора. Но после этого обычные наркотические симптомы проявились и в конечном итоге привели к смертельному исходу. [2559] Однако возбуждение от опьянения не всегда имеет эффект приостановки действия опиума; ибо в случае, который попал в мое поле зрения в лазарете этого города — случай женщины, которая проглотила полторы унции лауданума, будучи сильно пьяной, — обычные наркотические симптомы полностью сформировались через час: и хотя вскоре после этого был применен желудочный насос, она скончалась менее чем через пять часов с момента проглатывания лауданума — те, кто присматривал за ней до того, как ее привезли в больницу, пренебрегли использованием надлежащих средств для поддержания ее в сознании.

5. Отравление лауданумом и сулемой. — Из всех случаев комбинированного отравления, с которыми я сталкивался, наиболее примечательным является случай, произошедший в Эдинбургском замке несколько лет назад, отравления лауданумом и сулемой. В этом случае человек, молодой солдат, проглотил примерно в одно и то же время две драхмы последней и пол-унции первого. У него сначала не было никаких бурных симптомов, указывающих на действие сулемы, что является крайне редким явлением. Впоследствии у него были частые позывы к дефекации и тенезмы, с кровавым стулом и всеми обычными явлениями сильной дизентерии, но без боли в животе, без болезненности даже при сильном надавливании, без рвоты, кроме как при использовании рвотных средств. На четвертый день началось сильное слюнотечение; и под влиянием этого и дизентерийного поражения он быстро истощился, однако не настолько, чтобы в день своей смерти, девятый после приема яда, он не мог немного пройтись по своей комнате без посторонней помощи. Он умер на суднах довольно неожиданно. Я, к сожалению, потерял оригинальные записи, которые у меня были об этом случае, и забыл, присутствовали ли сначала какие-либо наркотические симптомы; но мое впечатление таково, что они присутствовали, хотя и в слабой степени. Большинство предыдущих подробностей были сообщены мне покойным д-ром Макинтошем. Желудок, двенадцатиперстная кишка, подвздошная кишка, толстая кишка и прямая кишка после смерти оказались в состоянии огромного воспаления, изъязвления и местами почти гангренозными. — В этом случае часть сулемы должна была быть разложена лауданумом, и образовался нерастворимый меконат ртути. Но количество, таким образом разложенное, могло составлять лишь малую долю от общего количества — как это было действительно доказано обширными разрушениями, фактически совершенными во всем пищеварительном канале. Поэтому я полагаю, что нет иного способа объяснить слабые видимые эффекты сулемы, особенно в начале, кроме как предположить, что наркотическое действие опиума завуалировало или фактически замедлило раздражающее действие сулемы.

6. Отравление опиумом и белладонной. — Дама, которая использовала сложный настой опиума и белладонны в качестве примочки при высыпаниях на вульве, однажды вздумала использовать эту примочку в качестве инъекции; и фактически получила три последовательные инъекции, содержащие каждая активное вещество скрупула опиума и пол-унции листьев белладонны. Она была тогда полностью без сознания и неподвижна, с бледным лицом, чрезмерно расширенными и не сокращающимися зрачками, частым и малым пульсом и учащенным дыханием. После использования слабительных инъекций, кровопускания, пиявок на голову и синаплизмов на ноги, она начала через пять часов проявлять некоторые признаки возвращающегося сознания, которые улучшились после приступа рвоты. Когда она полностью пришла в себя, ее зрение оставалось туманным, зрачки чрезмерно расширенными, а мысли несколько спутанными. В течение трех дней пульс оставался частым, а зрачки несколько расширенными. [2560] Здесь опиум, по-видимому, предотвратил бред, обычно вызываемый белладонной на ранней стадии, в то время как, с другой стороны, белладонна предотвратила обычный эффект опиума на зрачки и фактически произвела противоположное действие.

7. В следующих случаях активных ядов, которым подверглись люди, было так много, что невозможно сказать, какой или сколько из них вызвали симптомы. Производитель красок руководил процессом, в котором кобальт, мышьяк, ртуть, нашатырь и азотная кислота подвергались нагреванию в матрасе, когда матрас внезапно лопнул и мгновенно выделился густой пар. Производитель, прежде чем успел убежать, упал без сознания; и хотя был быстро удален, он умер через непродолжительное время, страдая от огромного вздутия живота. Рабочий, который также присутствовал, спасся через окно; но, тем не менее, у него немедленно началось вздутие живота, которое быстро стало очень большим и сопровождалось болью в челюстях и помутнением зрения. Эти симптомы очень медленно рассеивались при использовании холодных ванн и слабительных, которые вывели огромное количество зловонного газа. [2561]

Это не единственные примеры комбинированного отравления, которые привлекли мое внимание. Но другие, которые я заметил, не описаны с достаточной точностью, чтобы стоило их цитировать. Однако приведенные примеры достаточны, чтобы показать, что яды противоположных качеств, данные примерно в одно и то же время в больших дозах, будут маскировать эффекты друг друга, или препятствовать им, или, возможно, даже предотвращать их таким образом, что делает такую комбинацию обстоятельств важным предметом исследования для судебно-медицинского токсиколога.

Вероятно, что модифицирующее влияние устанавливается одним из двух способов — либо один яд вызывает состояние венозной плеторы или растяжения, которое препятствует или на время предотвращает всасывание другого, — либо один яд вызывает нечувствительность мембраны, с которой контактирует другой; так что не только местное повреждение, фактически нанесенное, не имеет обычного отдаленного эффекта на конституцию или на отдаленные органы, но также временами существенно менее обширно, чем в обычных обстоятельствах. ОПИСАНИЕ ТАБЛИЦЫ.

1. Маленькая воронкообразная трубка для тестирования мельчайших порций жидкостей.

2. Аппарат для перегонки жидкостей, подозреваемых в содержании кислот, в одну седьмую натуральной величины.

3. Трубка для восстановления очень малых порций мышьяка или ртути. Рисунок в половину натуральной величины. Шарик можно выдуть больше, если восстанавливаемый материал объемист.

4. Маленькая стеклянная воронка для введения материала в трубку Рис. 1, не загрязняя ее внутреннюю часть.

5. Обычный аппарат для выделения сероводорода. Воронка должна быть немного длиннее выходящей трубки. Жидкость ни в коем случае не должна быть намного выше, чем на рисунке, чтобы обезопасить оператора от ее вспенивания в выходящую трубку. Рисунок в одну четвертую натуральной величины.

6. Инструмент для смывания скудных осадков на фильтрах. Это тонкая бутылка, способная выдерживать нагрев — наполовину наполненная водой, которую при необходимости можно кипятить, — и имеющая пробку, пронзенную маленькой трубкой, вытянутой на внешнем конце до очень тонкого отверстия. Дыхание нагнетается в бутылку, и, когда бутылка затем переворачивается, очень тонкая струя выходит с большой силой.

7. Трубки натуральной величины для сбора малых порций ртути по процессу, стр. 300.

8. Пипетка, в одну четвертую натуральной величины, для удаления путем всасывания жидкостей, лежащих над осадками. Некоторые имеют прямоугольный изгиб в верхней части, с помощью которого оператор лучше видит кончик инструмента во время работы; но такие пипетки трудно чистить. Та, что представлена на рисунке, легко чистится пером.

9. Аппарат для восстановления сульфуретов некоторых металлов потоком водорода. A, сосуд с цинком и разбавленной серной кислотой, последнюю из которых можно восполнять через воронку B.
