Симптомы. — Они могут относиться к наркотическому или раздражающему типу. Обычно, однако, наблюдаются сильные колики с жаждой, рвотой и диареей, психическое возбуждение, сменяющееся бредом, судорогами, комой, редким пульсом, хриплым дыханием, цианозом, холодными конечностями и расширенными зрачками.
Патологоанатомическая картина. — При отравлении фаллином кровь остается жидкой; присутствуют многочисленные кровоизлияния, наблюдается жировая дегенерация внутренних органов.
Лечение. — Использовать желудочный зонд для введения раствора перманганата калия, рвотные средства с последующей подкожной инъекцией 1/50 грана атропина. Переливание солевого раствора. Доза касторового масла будет полезна.
Пищевые продукты. — Виды пищи, которые чаще всего вызывают симптомы отравления: свинина, телятина, говядина, мясные пироги, паштеты и консервированное мясо, сосиски и зельц. Отравление сосисками распространено в Германии. Для возникновения отравления необязательно, чтобы пища была «испорченной». Оно может быть следствием употребления мяса животного, страдавшего каким-либо заболеванием, заражения микроорганизмами или присутствия токсоальбуминов либо птомаинов. Многие заболевания, такие как диарея, брюшной тиф, холера и, возможно, туберкулез, могут быть вызваны употреблением зараженной пищи. Можно также упомянуть трихинеллез. Консервированная рыба часто вызывает симптомы отравления, а моллюски нередко бывают загрязнены патогенными микроорганизмами. Отравление мидиями некогда считалось обусловленным содержанием в них меди, попадающей с днищ кораблей, но оно гораздо вероятнее является результатом образования токсина при жизни, а не после начала разложения. Молоко также может вызывать раздражение желудочно-кишечного тракта из-за происходящих в нем химических изменений. Наблюдались эпидемии отравления от употребления сыра, содержащего тиротоксикон. Эрготизм от употребления хлеба, испеченного из зараженной спорыньей пшеницы, в настоящее время встречается редко, но пеллагра от употребления плесневелой кукурузы и латиризм, обусловленный примесью семян определенных видов вики к муке, все еще широко распространены в Южной Европе.
Симптомы. — Симптомы, возникающие в результате проглатывания ядовитого мяса, часто бывают очень тяжелыми. В некоторых случаях их появление задерживается на двадцать четыре — сорок восемь часов. Они могут напоминать симптомы инфекционного заболевания или острого энтерита. Обычно отмечаются головная боль, потеря аппетита, озноб, расстройство кишечника, боли в спине и конечностях, а также бред. Иногда симптомы напоминают отравление атропином — состояние, обусловленное птоматропином.
Лечение. — Рвотные средства, слабительные, стимуляторы, с подкожными инъекциями стрихнина и атропина наряду со стимуляторами.
XLVIII. — ПТОМАИНЫ, ИЛИ ТРУПНЫЕ АЛКАЛОИДЫ
Каждый врач, прежде чем выступать в качестве эксперта-свидетеля по делу о предполагаемом отравлении, должен досконально ознакомиться с новейшими исследованиями по данному вопросу. Птомаины представляют собой по большей части алкалоиды, образующиеся в процессе гниения, и они тесно напоминают многие растительные алкалоиды — например, вератрин, морфин и кодеин — не только по своим химическим свойствам, но и по физиологическим характеристикам. Вероятно, они родственны нейрину, алкалоиду, получаемому из мозга, а также из желчи. Некоторые из них аналогичны по своему действию мускарину — действующему началу мухомора. Некоторые из них являются протеинами, альбуминами и глобулинами. Птомаины могут в изобилии образовываться в веществах животного происхождения, которые после пребывания в антисанитарных условиях были изолированы от доступа воздуха. Птомаины или токсоальбумины иногда обнаруживаются в мясных паштетах и сосисках и обусловлены микроорганизмами: Bacillus botulinus, B. enteritidis Гертнера, B. proteus vulgaris или B. ærtrycke (который, пожалуй, является наиболее распространенным из всех). Симптомы, вызываемые последним, обычно включают рвоту, боли в животе, боли в конечностях и судороги, диарею, головокружение, зябкость, обморочное состояние и коллапс. Симптомами ботулизма являются сухость кожи и слизистых оболочек, расширение зрачков, паралич мышц, диплопия и т. д. Продуктами питания, с которыми чаще всего связано отравление, являются свинина, окорок, бекон, телятина, печеный мясной пирог, молоко, сыр, мидии, консервированное мясо.
В случае предполагаемого отравления адвокат защиты, если он хорошо знает свое дело, вероятно, подвергнет медицинского эксперта перекрестному допросу по этому вопросу и попытается добиться признания того, что реакции, которые были приписаны яду, возможно, можно объяснить теорией образования птомаина. Против этой формы защиты практически нет контрмер.
