Дымом от горящего древесного угля пользуются в целях самоубийства — метод, часто используемый на континенте, но практически неслыханный в Англии. Самоубийца обычно запирается в комнате, которую он изолировал от любой вентиляции и в которой поместил емкость с тлеющим коксом или древесным углем.

Отравление угарным газом происходит в двух формах — острой и хронической.

Симптомы: острые. — Первыми симптомами могут быть признаки возбуждения, за которыми быстро следуют сильная головная боль, головокружение, пульсация в висках и тошнота, переходящая в рвоту. Возникает мышечная слабость, утрачиваются чувствительность и рефлексы, следуют сонливость и кома, а в смертельных случаях перед смертью часто наступают судороги. Пульс на лучевой артерии становится неразличимым. Конъюнктива становится гиперемированной, глаза вытаращенными, зрачки расширенными и не реагирующими на свет. Произвольные и непроизвольные мышцы расслаблены, кожа холодная и цианотичная, губы покрыты пеной.

Хронические. — Симптомами являются головная боль, невралгические боли, анемия, одышка и истощение; на поздних стадиях они представляют собой признаки периферического неврита и психических расстройств.

Менее выраженные симптомы хронического отравления угарным газом нередки и возникают у тех, кто находится в маленьких и плохо проветриваемых помещениях, где могут быть отопительная печь, газовая печь или дефектная газовая арматура; последняя особенно опасна, когда для освещения используется водяной газ, поскольку он содержит высокий процент угарного газа.

Это очень сильный газ, быстро вызывающий смерть, действуя главным образом на нервную систему, причем симптомы соответствуют таковым при действии чистого наркотика.

Посмертные признаки — краснота лица с красноватыми пятнами на различных частях тела. Кровь — и это главное характерное свойство отравления угарным газом — вишнево-красная, что обусловлено химическим соединением, образующимся в результате действия газа на красящее вещество крови, парализующим таким образом кислородтранспортную способность кровяных телец. Предполагается, что газ соединяется с гемоглобином, образуя прочное соединение, причем спектроскопическое исследование показывает две полосы поглощения гемоглобина ближе к фиолетовому концу спектра, чем в нормальных условиях. Эти полосы напоминают полосы O₂Hb, поэтому их положение необходимо сравнить со спектром O₂Hb, расположив два спектра бок о бок.

Однако имеется другое важное различие, определяемое действием восстановителя, такого как сульфид аммония. Полосы COHb остаются неизменными, в то время как полосы O₂Hb уменьшаются. Смерть часто наступает до того, как весь Hb превратился в COHb, так что кровь содержит смесь COHb и O₂Hb, и при добавлении восстановителя спектр представляет собой составной спектр COHb и восстановленного Hb. Видима лишь широкая полоса восстановленного Hb, если количество присутствующего COHb составляет менее 28 процентов. В атмосфере, содержащей большой процент угарного газа, смерть может наступить до того, как кровь будет содержать достаточное количество COHb для появления характерного спектра.

Лечение заключается в удалении пострадавшего на свежий воздух, искусственном дыхании, венесекции и переливании артериализированной дефибринированной крови. Следует назначать ингаляции кислорода. В двух случаях за подкожными инъекциями нитроглицерина последовало выздоровление.

ВОДЯНОЙ ГАЗ

Этот газ получают путем пропускания водяного пара через раскаленный углерод и представляет собой соединение примерно равных частей окиси углерода и водорода. Своими опасными свойствами он обязан первому из названных газов. Когда водяной газ, чистый и простой, подается для целей отопления, его утечку невозможно обнаружить, так как газ не имеет запаха. Когда он используется для освещения и карбюрируется, его утечка легче обнаруживается по запаху, но даже в этом случае он опаснее каменноугольного газа, поскольку доля CO в нем выше. Несколько смертельных случаев произошло в результате использования водяного газа для целей отопления и освещения, а также для плавки стали в Лидсе. Симптомы отравления соответствуют отравлению угарным газом.

СЕРОВОДОРОД

Сероводород — это газ, обладающий сильным запахом тухлых яиц. Он широко используется в качестве реактива для большинства металлов; и его присутствие можно обнаружить по фильтровальной бумаге, смоченной солью свинца, которая чернеет.

Симптомы. — При умеренном разбавлении газа возникают симптомы: головокружение, пульсация в висках, боль и чувство тяжести в желудке, тошнота и рвота; иногда наблюдаются бред и судороги, а также затрудненное дыхание и нерегулярный пульс. Когда газ разбавлен слабо, человек внезапно слабеет, теряет сознание и быстро умирает.

Посмертные признаки. — Жидкое состояние и чернота крови, потеря мышечной сократимости и склонность к быстрому гниению. Бронхиальные трубки покрасневшие, а внутренние сосудистые органы могут казаться почти черными.

Лечение. — Оно будет заключаться в немедленном удалении человека на свежий воздух и введении стимуляторов, а также в дыхании газообразным хлором, выделяемым из хлорной извести под действием кислоты.

КАМЕННОУГОЛЬНЫЙ ГАЗ

Каменноугольный газ состоит из нескольких углеводородов, главным из которых является болотный газ, а также свободного водорода, угарного газа и углекислого газа, аммиака, сероводорода и сульфидов углерода, которые придают ему особый запах. Ядовитые свойства каменноугольного газа обусловлены угарным газом, которого содержится 7,5 процента в обычном газе, поставляемом для освещения. Его можно обнаружить путем пропускания каменноугольного газа через кислый раствор хлористой меди, который чернеет в результате образования соединения CuCOCl. Опасное взрывчатое соединение образуется, когда доля газа достигает 1 к 10 воздуха. Отравление этим газом, как правило, носит случайный характер.

Симптомы. — Головная боль, тошнота, рвота, головокружение, переходящее в кому. В некоторых случаях отмечается храпящее дыхание. Если пострадавшего удалить от газа, от его дыхания сильно пахнет газом. Убийца Шантрель пытался скрыть свое преступление, пустив газ в спальню своей жены, но попытка не удалась. Зрачки перед смертью, как правило, расширены.

Посмертные признаки. — Вишнево-красный цвет крови, покраснение легочной ткани и пена в дыхательных путях. Сосуды мозга переполнены кровью, на бедрах появляются розовые пятна.

Лечение. — Оно заключается в удалении пациента на свежий воздух, искусственном дыхании, ингаляциях кислорода и т. д., как при отравлении угарным газом.

ГАЗЫ ОТ СГОРАНИЯ

Токсические эффекты производились вдыханием газов, образующихся при взрывчатых веществах. Основными газами являются углекислый газ, угарный газ и азот. Порох выделяет значительное количество CO и сероводорода. Нитроглицерин, динамит и пироксилин выделяют большое количество CO. Тонит выделяет очень мало CO, а робурит — ни одного. Бездымные пороха выделяют CO.

Производство «робурита» и «зихерхета», содержащих динитробензин, чревато опасностью от этого вещества, вызывающего в острых случаях цианоз лица или всего тела, головную боль, головокружение, парез, зябкость, частый пульс, одышку, поверхностное дыхание с большими интервалами и кому. Может наблюдаться рвота, а кровь приобретает шоколадный цвет. Хроническая форма отравления вызывает синюшность и цианоз с гастритом, увеличением печени и желтухой, парестезией, онемением и судорогами в мышцах, амблиопией с концентрическим сужением полей зрения и центральной скотомой. Кровь похожа на таковую при пернициозной анемии, а моча коричневая или черноватая.

АЦЕТИЛЕН

Этот газ имеет характерный запах герани. Он является продуктом неполного сгорания углеводородов и образуется при горении ламп или газовых рожков при недостатке воздуха, например в горелке Бунзена, в которой «произошел проскок пламени», а также при использовании масляных печей и газовых кухонных и отопительных приборов. Он используется для освещения и образуется при действии воды на карбид кальция. Он образует с воздухом взрывоопасную смесь. Он не является мощным ядом. Продолжительное воздействие его вызывает анемию, нарушение питания и нервные симптомы. У животных он вызывает наркоз. Он не соединяется с гемоглобином, а действует как индифферентный газ.

МОНООКСИД АЗОТА, ЗАКИСЬ АЗОТА

Это вещество известно как веселящий газ; оно имеет сладковатый вкус и запах. При вдыхании в небольших количествах оно вызывает покалывание и побуждает к смеху, отсюда и его название. При вдыхании в анестетических целях кожа становится синюшной, кровяное давление повышается, наступает потеря сознания. Оно действует сначала на высшие нервные центры, затем на спинной мозг, продолговатый мозг и сердце. Если переусердствовать, оно вызывает смерть от асфиксии. При обычном использовании для анестезии последняя наступает быстро, а после прекращения введения наступает быстрое восстановление. Оно оказывает специфическое действие на некоторых людей, которые могут не только проявлять симптомы веселья, но в некоторых случаях становиться крайне агрессивными.

ПАРЫ БЕНЗИНА

Пары бензина оказывают токсическое действие на тех, кто их вдыхает, включая извращение вкуса, дисфагию, головную боль, головокружение, цианоз, потерю чувствительности, манию и слабоумие. Во время выздоровления возникают приступы мании. Может последовать периферический неврит.

НАФТА, БЕНЗОЛ ИЛИ БЕНЗОЛ

Это вещество вызывает отравление при проглатывании или вдыхании, например при чистке перчаток, гидроизоляции и т. д. Смерть наступала в обоих случаях.

Симптомы. — При остром отравлении наблюдаются возбуждение, прилив крови к лицу, цианоз, расширенные зрачки, головная боль, замедленное дыхание, ступор и кома с желудочно-кишечным раздражением.

У работающих с ним могут возникать галлюцинации и бред; определенную роль играет идиосинкразия. Женщины могут становиться опьяненными, возбужденными и истеричными. Он может вызывать головную боль, головокружение, наркоз и неспособность ходить, сопровождающиеся рвотой. Могут возникать небольшие кровотечения. При высокой концентрации паров может наступить быстрая кома и смерть.

Лечение. — При проглатывании следует использовать желудочный зонд и вводить подкожно эфир и стрихнин. При отравлении парами требуются вынос на свежий воздух, искусственное дыхание, ингаляции кислорода и общеукрепляющие средства.

СЕРНИСТЫЙ ГАЗ

Это непригодный для дыхания газ с запахом горящей серы. Он является консервантом и отбеливающим средством, используется для дезинфекции и применяется в некоторых отраслях промышленности.

Вдыхание паров вызывает ощущение удушья со спазмом голосовой щели и раздражением носа, трахеи и бронхов, вызывая чихание и кашель; могут наблюдаться помутнение роговицы, одышка, цианоз и судороги.

ХЛОР

Газ используется для дезинфекции и отбеливания. На химических заводах может возникать хроническое отравление, вызывающее анемию, истощение, гастрит, кариес зубов, бронхит и эмфизему. В концентрированном виде он вызывает одышку, сильный кашель, кровохарканье, ступор и обморок.

Лечение. — Свежий воздух, ингаляции паров и общее лечение легочных состояний.

ФОСФОРИСТЫЙ ВОДОРОД

Это очень ядовитый газ. Смертельные случаи происходили на борту судов, перевозивших грузы электролитического ферросилиция, содержащего фосфид кальция. Он восстанавливает оксигемоглобин. Он может вызвать быстрое, сменяющееся медленным и затрудненным дыхание и судороги.
